REPORT OF DEATH (ROD)

The death of a Filipino national in Colombia, Ecuador, or Venezuela should be reported to the Embassy in order to have a record with the Philippine Statistics Authority (PSA) in the Philippines.

Requirements:

  1. Four (4) duly accomplished Report of Death forms 
  2. Original and four (4) photocopies of the death certificate with English translation and Apostille 
  3. Original passport of the deceased

 

Requirements for Shipment of Human Remains to the Philippines:

  1. Certificate of death issued by the local civil registrar, or similar authority, showing the name, age and nationality of the deceased person, as well as the place, date and cause of death, as certified by the attending physician. The certificate must be with English translation and Apostille.
  2. Notarized certificate issued by the Mortuary/Funeral authority stating that the body was properly embalmed/cremated in accordance with internationally accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased. The certificate must be with English translation and Apostille.
  3. Certificate of Non-contagious Disease issued by the proper health authority, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease. The certificate must be with English translation and Apostille.
  4. A transit permit issued by the proper health authority. The permit must be with English translation and Apostille.

Consular Mortuary Certificate

Note: The Embassy personnel may require additional documents depending on the circumstances of the applicant.
