REPORT OF BIRTH (ROB)

A Report of Birth must be filed with the Philippine Embassy in Bogota for a child born of a Filipino father or a Filipino mother in Colombia, Ecuador or Venezuela. A Report of Birth Form must be filed within thirty (30) days from the birth of the child. Filing a Report of Birth beyond the 30 – day period will be classified as delayed registration of birth which is subject to a different set of documentary requirements.  Please note that the Philippine Embassy may require additional documents in support of the Filipino citizenship of the child. Requirements if parents are married A.  Report of Birth
  1. Four (4) original Report of Birth (ROB) Forms duly accomplished by the parents to be signed in the presence of a consular officer. The ROB Form may be downloaded from the Embassy website.
B.  Birth Certificate
  1. One original or Certified True Copy of the Birth Certificate (BC) (the original BC will  be returned after processing) plus four (4) photocopies of the same document.
  2. Both original and photocopies of the BC (item a) should be duly apostilled by the Ministry of Foreign Affairs of the country of birth. 
  3. If the BC is in Spanish, an English translation of the BC duly certified by an official translator is required. 
C.  Marriage Certificate
  1. One original or Certified True Copy of the Marriage Certificate (MC) of the parents (the original MC will be returned after processing) plus  four (4) photocopies of the same document.
D.  Passports
  1. Original valid passports of both Filipino parents (the original will be returned after processing) plus four (4) photocopies of each of their passports 
  2. If the original valid passport cannot be presented, the Filipino parent has to submit at least one valid Philippine government-issued ID bearing the parent´s photograph and signature
  3. For a non-Filipino parent,  original valid passport (the passport will be returned after processing) or in lieu thereof, a valid government-issued ID bearing the parent´s photograph and signature, plus four (4) photocopies of the same document.
E.  Order of Approval or Petition for Reacquisition of Philippine Citizenship
  1. For parents with reacquired Philippine citizenship, four (4) photocopies of the Order of Approval or Petition for Reacquisition of Philippine Citizenship indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship.
Requirements if the parents are not married
  1. Requirements listed as Nos. 1, 2, and 4 above such as Report of Birth Form, Birth Certificate, Passports of parents
  2. Notarized Affidavit of Acknowledgment of Paternity executed by the biological father
  3. Notarized Affidavit to Use the Surname of the Father  – to be executed based on the following conditions during the time of the application of the Report of Birth:
    • For applicants below 6 years old – to be executed by the mother or guardian
    • For applicants between 7 and 17 years old – to be executed by the child plus Sworn Attestation of the mother or guardian
    • For applicants 18 years of age and above – the applicant will execute the Affidavit to Use the Surname of the Father
  4. Notarized Affidavit of Illegitimacy to be executed by the mother if the child will use the mother’s maiden name
  5. If one parent is a foreigner or non-Filipino, original foreign passport (the passport will be returned after processing) or in lieu thereof, a valid government-issued ID bearing the photograph and signature of the parent, plus  four (4) photocopies of the passport or the ID. 
For parents with  dual citizenship, four (4) photocopies of the Approved Petition Under Oath to Re-acquire/Retain Philippine Citizenship Under R.A. 9225 indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship, and Order of Approval
Fees:
Registration of Birth US$ 25.00 (or its equivalent amount in COP based on current Embassy exchange rate)
Affidavit US$ 25.00 each (or its equivalent amount in COP based on current Embassy exchange rate)
Note: The Embassy personnel may require additional documents depending on the circumstances of the applicant. DELAYED REGISTRATION OF BIRTH Requirements if parents are married A. Report of Birth Form
  1. Four (4) original Report of Birth Forms duly accomplished by the parents 
B.  Affidavit of Delayed Registration 
  1. Notarized Affidavit of Delayed Registration executed by one parent
C.  Affidavit of Two Disinterested Persons
  1. If the child is more than one year old, notarized Affidavit of Two Disinterested Persons who have witnessed or who have knowledge of the birth of the child 
D.  Birth Certificate
  1. One (1) original or Certified True Copy of the Birth Certificate (BC) duly apostilled by the Ministry of Foreign Affairs of the country of birth plus four (4) photocopies of the same document
  2. If the BC is in Spanish, an English translation of the BC duly certified by an official translator  
E.  Marriage Certificate
  1. One (1) original or Certified True Copy of the Marriage Certificate of the parents plus four (4) photocopies of the same document (the original MC will be returned after processing)
F.  Passports
  1. Original valid passports of both Filipino parents (the original will be returned after processing) plus four (4) photocopies of each of their passports 
  2. If a Filipino parent does not have the original valid passport, the concerned parent has to submit at least one valid Philippine government-issued ID bearing his or her photograph and signature
G.  Petition for Reacquisition of Philippine Citizenship and Order of Approval
  1. For parents with  dual citizenship, four (4) photocopies of the Approved Petition Under Oath to Re-acquire/Retain Philippine Citizenship Under R.A. 9225 indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship, and Order of Approval
Requirements if the parents are not married
  1. Items 1, 2, 3 and 4 above
  2. Present original Philippine passport and submit four (4) photocopies. If no valid Philippine passport, at least one valid identification document issued by a Philippine government agency bearing the photograph and signature of the individual.
  3. Notarized Affidavit of Acknowledgment of Paternity and Affidavit to Use Father’s Surname 
  4. Affidavit of Illegitimacy if the child will use the mother’s maiden name. 
  5. If one parent is a foreigner, present original foreign passport or government-issued ID, and submit four (4) photocopies
  6. For parents with  dual citizenship, four (4) photocopies of the Approved Petition Under Oath to Re-acquire/Retain Philippine Citizenship Under R.A. 9225 indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship, and Order of Approval
