NBI Clearance Application Form

Steps to apply for an NBI Fingerprint Capturing: 

 

  1. Schedule a visit to the Embassy by booking an appointment at
    bit.ly/consular-appointment;
  2. The applicant will undergo the fingerprinting process at the Embassy.  
  3. The Embassy will notarize the NBI Clearance Application Form;
  4. The applicant will also execute a Special Power of Attorney for his relative

who will process the filing of the application with the NBI;

  1. The applicant will send the Embassy-notarized NBI application form and

SPA to his relatives in the Philippines;

  1. The representative will submit the NBI Application Form to an NBI office for the issuance of NBI Clearance.  Upon release of the NBI clearance, the designated representative will bring the NBI clearance to the DFA for Apostille; and
  2. The NBI clearance with Apostille will be sent to the applicant to its intended
    destination. Translation into Spanish language will depend on the requirement 

of the requesting office/agency in Colombia. 



Note: Alternatively, you may also directly apply for the renewal of your NBI Clearance on the National Bureau of Investigation’s website at https://clearance.nbi.gov.ph if it was issued from 2014 to the present.
