
PASSPORT APPLICATION
Requirements:
- Appointment confirmation through appointment system
bit.ly/consular-appointment - Duly accomplished application form
- Current Philippine passport – original and one (1) photocopy of the passport information/data page
- Payment – USD 60 (or its equivalent to COP)
For First-time Minor applicants (below 18 y/o):
- PSA-issued Birth Certificate (BC), if the applicant was born in the Philippines – original and one (1) photocopy
- PSA-issued Report of Birth (for those born overseas)/Report of Birth issued by the Philippine Embassy/Consulate General – original and one (1) photocopy
(Local Civil Registrar copy may be required if PSA certificate of Live Birth is not clear or cannot be read.)
- Marriage Certificate or Report of Marriage of parents – original and one (1) photocopy
- Valid passport of parents – original and one (1) photocopy
- Minor child should be accompanied by either the mother or the father (for unmarried parents, the mother should accompany the child)
For newly-married women who will use their husband’s surname:
- If married in the Philippines, original Marriage Certificate (MC) issued by the Philippine Statistics Authority (PSA) with Apostille Certificate issued by the Department of Foreign Affairs – original and one (1) photocopy
- A transcribed MC from the Local Civil Registry is required if the entries in the PSA issued MC is/are blurred or unreadable – original and one (1) photocopy
For women who would like to revert to their maiden name (by virtue of death of the spouse):
- Death Certificate of the deceased spouse, issued by the Philippine Statistics Authority (PSA) with Apostille Certificate issued by the Department of Foreign Affairs (DFA) – original and one (1) photocopy
- If spouse died in Colombia or other countries, Death Certificate in English authenticated/Apostilled by the Ministry of Foreign Affairs (or equivalent) in the country of death – original and one (1) photocopy
- PSA-issued Certificate of Live Birth or Report of Birth
- Latest issued Philippine passport (if available)
For women who would like to revert to their maiden name (by virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
- PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
- PSA-issued Certificate of Live Birth or Report of Birth
- Latest issued Philippine passport
For dual citizens:
- Natural-born dual citizens
- Latest issued Philippine passport
- PSA-issued Certificate of Live Birth or PSA-issued Report of Birth
- Dual Citizens who availed of the provisions of Republic Act No. 9225
- Latest issued Philippine passport
- Original and photocopy of Oath of Allegiance, Order of Approval, Identification Certificate issued by a Philippine Foreign Service Post; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by the Bureau of Immigration
- Valid government-issued ID or their secondary citizenship/residence counterparts
For naturalized Filipino citizens / those who obtained Filipino citizenship by election:
- Identification Certificate / Certificate of Naturalization issued by the Bureau of Immigration (BI) / relevant Philippine authority – original and photocopy
- Affidavit of Election of Philippine citizenship (if applicable)
PASSPORT RELEASING
All passports are printed in the Philippines. New passports are expected to be available after 4-6 weeks after a successful application process.
Requirements for passport releasing:
- Official receipt
- Current Philippine passport, which will be canceled and returned to the applicant together with the new passport.
- Passports to be claimed by immediate family – duly signed authorization letter from the passport holder and proof of identification of the family member.
- Passports to be claimed by non-family members – Special Power of Attorney (SPA) from the passport holder and proof of identification of the person claiming the passport.
- Passports of minors can be claimed only by the Filipino mother/father.
