PASSPORT APPLICATION

Requirements:

  1. Appointment confirmation through appointment system
    bit.ly/consular-appointment
  2. Duly accomplished application form
  3. Current Philippine passport – original and one (1) photocopy of the passport information/data page
  4. Payment – USD 60 (or its equivalent to COP)

For First-time Minor applicants (below 18 y/o):

  1. PSA-issued Birth Certificate (BC), if the applicant was born in the Philippines – original and one (1) photocopy
  2. PSA-issued Report of Birth (for those born overseas)/Report of Birth issued by the Philippine Embassy/Consulate General – original and one (1) photocopy

(Local Civil Registrar copy may be required if PSA certificate of Live Birth is not clear or cannot be read.)

  1. Marriage Certificate or Report of Marriage of parents – original and one (1) photocopy
  2. Valid passport of parents – original and one (1) photocopy
  3. Minor child should be accompanied by either the mother or the father (for unmarried parents, the mother should accompany the child)

For newly-married women who will use their husband’s surname:

  1. If married in the Philippines, original Marriage Certificate (MC) issued by the Philippine Statistics Authority (PSA) with Apostille Certificate issued by the Department of Foreign Affairs – original and one (1) photocopy
  2. A transcribed MC from the Local Civil Registry is required if the entries in the PSA issued MC is/are blurred or unreadable – original and one (1) photocopy

For women who would like to revert to their maiden name (by virtue of death of the spouse):

  1. Death Certificate of the deceased spouse, issued by the Philippine Statistics Authority (PSA) with Apostille Certificate issued by the Department of Foreign Affairs (DFA) – original and one (1) photocopy
  2. If spouse died in Colombia or other countries, Death Certificate in English authenticated/Apostilled by the Ministry of Foreign Affairs (or equivalent) in the country of death – original and one (1) photocopy
  3. PSA-issued Certificate of Live Birth or Report of Birth
  4. Latest issued Philippine passport (if available)

For women who would like to revert to their maiden name (by virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):

  1. PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
  2. PSA-issued Certificate of Live Birth or Report of Birth
  3. Latest issued Philippine passport

For dual citizens:

  • Natural-born dual citizens
    1. Latest issued Philippine passport
    2. PSA-issued Certificate of Live Birth or PSA-issued Report of Birth
  • Dual Citizens who availed of the provisions of Republic Act No. 9225
    1. Latest issued Philippine passport
    2. Original and photocopy of Oath of Allegiance, Order of Approval, Identification Certificate issued by a Philippine Foreign Service Post; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by the Bureau of Immigration
    3. Valid government-issued ID or their secondary citizenship/residence counterparts

For naturalized Filipino citizens / those who obtained Filipino citizenship by election:

  1. Identification Certificate / Certificate of Naturalization issued by the Bureau of Immigration (BI) / relevant Philippine authority – original and photocopy
  2. Affidavit of Election of Philippine citizenship (if applicable)

PASSPORT RELEASING

All passports are printed in the Philippines. New passports are expected to be available after 4-6 weeks after a successful application process.

Requirements for passport releasing:

  1. Official receipt
  2. Current Philippine passport, which will be canceled and returned to the applicant together with the new passport.
  3. Passports to be claimed by immediate family – duly signed authorization letter from the passport holder and proof of identification of the family member.
  4. Passports to be claimed by non-family members – Special Power of Attorney (SPA) from the passport holder and proof of identification of the person claiming the passport.
  5. Passports of minors can be claimed only by the Filipino mother/father.
