PASSPORT APPLICATION
REQUIREMENTS
- Confirmed appointment secured through bit.ly/consular-appointment
- Duly accomplished passport application form
- Current Philippine passport – original and one (1) photocopy of the passport information/data page
- Payment – USD 60 (or its equivalent to COP)
For First-time Minor applicants (below 18 y/o):
- PSA-issued Birth Certificate (BC), if the applicant was born in the Philippines – original and one (1) photocopy
- PSA-issued Report of Birth (for those born overseas) / Report of Birth issued by the Philippine Embassy/Consulate General – original and one (1) photocopy (Local Civil Registrar copy may be required if PSA certificate of Live Birth is not clear or cannot be read.)
- PSA-issued Marriage Certificate / Report of Marriage of parents / Foreign Marriage Certificate with Apostille – original and one (1) photocopy
- Valid passport of parents – original and one (1) photocopy
- For married parents, minor child should be accompanied by the mother or the father
- For unmarried parents, only the mother may accompany the child. (In the absence of the mother, the minor child may be accompanied by the legal guardian or the person exercising substitute parental authority)
For newly-married women who will use their husband’s surname:
- If married in the Philippines, original and one (1) photocopy of the PSA-issued Marriage Certificate (MC) with DFA Apostille (A transcribed MC from the Local Civil Registry is required if the entries in the PSA issued MC is/are blurred or unreadable – original and one (1) photocopy)
- If married abroad, original Report of Marriage (ROM) and one (1) photocopy
For women who would like to revert to their maiden name (by virtue of death of the spouse):
- Original and one (1) photocopy of the PSA-issued Death Certificate of the deceased spouse with DFA Apostille
- If the spouse died in Colombia or other countries, original and one (1) photocopy of the Death Certificate with Authentication/Apostille and English translation
- Original and one (1) photocopy of the PSA-issued Certificate of Live Birth or Report of Birth
- Latest issued Philippine passport
For women who would like to revert to their maiden name (by virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
- Original and one (1) photocopy of the PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
- Original and one (1) photocopy of the PSA-issued Certificate of Live Birth or Report of Birth
- Latest issued Philippine passport
For dual citizens:
- Natural-born dual citizens
- Latest issued Philippine passport
- PSA-issued Certificate of Live Birth or PSA-issued Report of Birth
- Dual Citizens who availed of the provisions of Republic Act No. 9225
- Latest issued Philippine passport
- Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine Foreign Service Post; or Certificate of Retention/Reacquisition of Philippine Citizenship or any equivalent document issued by the Bureau of Immigration
- Valid government-issued ID or their secondary citizenship/residence counterparts
For naturalized Filipino citizens / those who obtained Filipino citizenship by election:
- Identification Certificate / Certificate of Naturalization issued by the Bureau of Immigration (BI) / relevant Philippine authority – original and photocopy
- Affidavit of Election of Philippine citizenship (if applicable)
REQUIREMENTS FOR THE RELEASE OF PASSPORT
- Official receipt
- Current Philippine passport, which will be canceled and returned to the applicant together with the new passport.
- Passports to be claimed by immediate family – duly signed authorization letter from the passport holder and proof of identification of the family member.
- Passports to be claimed by non-family members – Special Power of Attorney (SPA) from the passport holder and proof of identification of the person claiming the passport.
- For married parents, passports of minors can be claimed only by the mother or father.
- For unmarried parents, passports of minors can be claimed only by the mother.
All passports are printed in the Philippines. New passports are expected to be available after 4-6 weeks after a successful application process.
LOST PASSPORT
REQUIREMENTS
- Personal appearance
- Passport application form
- Affidavit of Loss of Passport
- Police Report
- Photocopy of the passport data page
- Lost passport fee – USD 150 or equivalent to COP on top of the regular passport fee
Notes:
- The Embassy may require additional documents from the applicant.
- Filipinos who lost their passports while visiting Colombia, Ecuador, or Venezuela, will be issued a Travel Document for direct one-way travel to the Philippines.
