REPORT OF DEATH (ROD)

The death of a Filipino national in Colombia should be reported to the Embassy in order to have a record with the Philippine Statistics Authority (PSA) in the Philippines. 

Requirements:

  1. Five duly accomplished Report of Death forms 
  2. Present original death certificate and submit three copies with English translation. The original death certificate will be returned to the next-of-kin or duly authorized representative
  3. Copy of passport of deceased

Requirements for Shipment of Remains to the Philippines:

  1. Certificate of death issued by the local civil registrar, or similar authority, showing the name, age and nationality of the deceased person, as well as the place, date and cause of death, as certified by the attending physician;
  2. A duly notarized certificate issued by the Mortuary/Funeral authority, stating that the body was properly embalmed/cremated in accordance with internationally accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased;
  3. Certificate of Non-contagious Disease issued by the proper health authority, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease; and
  4. A transit permit issued by the proper health authority.

US$ 25.00 (or its equivalent amount in COP based on current Embassy exchange rate)

Report of Death

Note: The Embassy personnel may require additional documents depending on the circumstances of the applicant.
