REPORT OF MARRIAGE
Marriages Solemnized in Colombia, Ecuador, or Venezuela:
- Duly accomplished Report of Marriage Form
- Original and four (4) photocopies of Apostilled Marriage Certificate with Official English Translation
- Passports or government-issued IDs of both parties valid at the time of marriage (four (4) photocopies of data pages)
- Notarized Affidavit of Delayed Registration (required only for applicants who are married for more than one (1) year
- Notarized Affidavit of Two (2) Disinterested Persons (required only for applicants who are married for more than one (1) year
- Original and four (4) photocopies of Negative Certification of Marriage or Certificate of No Marriage Record (CENOMAR) from the Philippine Statistics Authority (PSA) (CRS Form No. 3)
ADDITIONAL REQUIREMENTS:
For Annulled or Legally Separated Filipino Spouse – submit copy of DFA Apostille Marriage Contract issued by the PSA with proper annotation;
For Divorced Filipino Spouse who has no marriage record in PSA – submit copy of Divorce Decree / Certificate recognized/validated by the Regional Trial Court in the Philippines with DFA Apostille;
For Widowed Filipino Spouse – submit copy of PSA-issued Death Certificate with DFA Apostille or Foreign death certificate of previous spouse with Authentication or Apostille of the issuing country;
For Widowed Foreign Spouse – submit copy of PSA-issued Death Certificate of previous spouse with DFA Apostille
*The requirements apply to marriages solemnized in Colombia, Ecuador, or Venezuela (1) between Filipinos, and (2) between a Filipino and a foreigner.
