REPORT OF MARRIAGE

Marriages Solemnized in Colombia, Ecuador, or Venezuela:

  1. Duly accomplished Report of Marriage Form
  2. Original and four (4) photocopies of Apostilled Marriage Certificate with Official English Translation 
  3. Passports or government-issued IDs of both parties valid at the time of marriage (four (4) photocopies of data pages)
  4. Notarized Affidavit of Delayed Registration (required only for applicants who are married for more than one (1) year
  5. Notarized Affidavit of Two (2) Disinterested Persons (required only for applicants who are married for more than one (1) year
  6. Original and four (4) photocopies of Negative Certification of Marriage or Certificate of No Marriage Record (CENOMAR) from the Philippine Statistics Authority (PSA) (CRS Form No. 3)

 

ADDITIONAL REQUIREMENTS:

For Annulled or Legally Separated Filipino Spouse – submit copy of DFA Apostille Marriage Contract issued by the PSA with proper annotation;

For Divorced Filipino Spouse  who has no marriage record in PSA – submit copy of Divorce Decree / Certificate recognized/validated by the Regional Trial Court in the Philippines with DFA Apostille;

For Widowed Filipino Spouse – submit copy of PSA-issued Death Certificate with DFA Apostille or Foreign death certificate of previous spouse with Authentication or Apostille of the issuing country;

For Widowed Foreign Spouse – submit copy of PSA-issued Death Certificate of previous spouse with DFA Apostille

*The requirements apply to marriages solemnized in Colombia, Ecuador, or Venezuela (1) between Filipinos, and (2) between a Filipino and a foreigner.
