REPORT OF BIRTH (ROB)

The birth of children born in Colombia of Filipino parent/s should be reported to the Philippine Embassy in Bogota within thirty (30) days from the date of birth. Registration beyond the thirty (30) days will be considered delayed registration of birth.

 

Requirements for Report of Birth: (Parents are married)

  1. Five (5) original Report of Birth forms duly accomplished by the parent/s. 
  2. One (1) original or certified true copy of Birth Certificate (BC) duly Apostille by the Ministry of Foreign Affairs (MFA) and submit four (4) photocopies (original will be returned).  If the BC is in Spanish, an official English translation (certified by an official translator) is required.  
  3. One (1) original or certified true copy and four (4) photocopies of Marriage Certificate of the parents (original will be returned).
  4. Present original valid Philippine passport and submit four (4) photocopies. If no valid Philippine passport, at least one valid identification document issued by a Philippine government agency bearing the photograph and signature of the individual.
  5. If one parent is a foreigner, present original foreign passport or government-issued identification card, and submit four (4) photocopies.
  6. For parents who are dual citizens, four (4) photocopies of Order of Approval or Petition for Reacquisition of Philippine Citizenship, indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship.

Requirements if the parents are not married:

  1. Items 1, 2, 3 and 4 above 
  2. Notarized Affidavit of Acknowledgment of Paternity executed by the biological father
  3. Affidavit to Use Father’s Surname executed by mother or child
  4. Affidavit of Illegitimacy if the child will use the mother’s maiden name. 
  5. If one parent is a foreigner, present original foreign passport or government-issued identification card, and submit four (4) photocopies.
  6. For parents who are dual citizens, four (4) photocopies of Order of Approval or Petition for Reacquisition of Philippine Citizenship, indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship.

Registration of Birth

US$ 25.00 (or its equivalent amount in COP based on current Embassy exchange rate)

Affidavit

US$ 25.00 each (or its equivalent amount in COP based on current Embassy exchange rate)

Note: The Embassy personnel may require additional documents depending on the circumstances of the applicant.

DELAYED REGISTRATION OF BIRTH

Requirements: (Parents are married)

  1. Five (5) original Report of Birth forms duly accomplished by the parent/s. 
  2. Notarized Affidavit of Delayed Registration. 
  3. If the child is more than one year old, notarized Affidavit of Two Disinterested Persons who have witnessed or known the birth of the child. 
  4. One (1) original or certified true copy of Birth Certificate (BC) duly Apostille by the Ministry of Foreign Affairs, and four (4) photocopies (original will be returned).  If the BC is in Spanish, an official English translation (certified by an official translator) is required.  
  5. One (1) original or certified true copy and four (4) photocopies of Marriage Certificate of the parents (original will be returned)
  6. Present original Philippine passport and submit four (4) photocopies. If no valid Philippine passport, at least one valid identification document issued by a Philippine government agency bearing the photograph and signature of the individual.
